The House of Hope Long-Term Treatment Program is seeking a case manager.
Job Duties and Responsibilities:
- Coordinates client assessments, treatment planning, and crisis intervention services
- Provides training and facilities linkage for usage of community resources
- Assists clients in obtaining access to necessary services as identified by clients and treatment team (e.g., vocational services, financial services, other healthcare services)
- Assists individuals in performing activities of daily living
- Provides clients with transportation to appointments
- Documents services to clients in the clinical record
- All other duties as assigned by supervisor
Skills needed:
Multi-tasking, attention to detail, organizational skills, electronic health record keeping, positive mental attitude, team player, problem solving skills, and strong interpersonal skills.
Qualifications:
Required: CDCA credential at a minimum
Preferred: Previous experience in a behavioral healthcare setting
Job Type: Full-time
Pay: $40,000.00 - $42,000.00 per year
