Job Summary
As an Assessment Outreach Coordinator, you will play a vital role in expanding our outreach and increasing our impact by partnering with hospitals, jails, and other referral organizations. Your primary responsibility will be conducting same-day assessments at jails, hospitals, and treatment centers, while also sharing and promoting the exceptional services offered by House of Hope. You will serve as a key liaison, facilitating access to care for patients in need. Your work will have a direct and meaningful impact on transforming lives and fostering healthier communities. This position is unique in that your main focus will be on clinical assessments, with community outreach as a secondary responsibility.
Duties
- Respond to assessment inquiries and requests in a timely and efficient manner.
- Provide same day assessments at jails, hospitals, treatment centers and other community locations.
- Participate in regular staff meetings and trainings as scheduled.
- Manage project timelines, ensuring all activities are completed on schedule.
- Focus on building and fostering new community relationships in efforts to raise awareness of House of Hope for Recovery.
- Aid in collection of data to inform assessing the effectiveness of outreach efforts.
- Collaborate with local organizations to enhance partnerships and resource sharing.
- Provide addiction counseling support as needed, ensuring a compassionate approach.
- Collaborate with internal teams to ensure a seamless and efficient patient referral process, demonstrating a patient-centric approach throughout.
- Aid in tracking and analyzing data related to business development efforts, identifying areas for improvement and growth.
Qualifications
- CDCA qualification (minimum requirement).
- Experience with substance use disorder clinical assessments (preferred).
- Familiarity with addiction counseling principles (preferred).
- Excellent leadership qualities with a commitment to community service.
- A degree in human services, social work, public health, or a related field (preferred).
- Valid Ohio driver's license, automobile (will use for travel related to work with mileage reibursement) and acceptable driving record are required; and
- Pre-employment criminal background check and drug testing are mandatory
- Exceptional communication and presentation skills, with the ability to articulate the value and mission of House of Hope for Recovery.
