What You’ll Do:
- Consult with patients and their families to understand their health needs.
- Educate patients about their condition, treatments, and medications.
- Develop tailored care plans that address their specific health requirements.
- Collaborate with healthcare providers and specialists to schedule appointments and treatments.
- Regularly check in on patients, monitor their progress, and document changes.
- Assist the care team in evaluating and improving health interventions.
- Stay updated on the latest healthcare practices through training.
- Help secure funding for patient care as needed.
- Treat every patient with empathy, respect, and professionalism.
- Ensure compliance with healthcare regulations and organizational standards.
What We’re Looking For:
- Minimum of 3 years’ experience in care coordination or clinical settings.
- Strong verbal and written communication skills.
- Proficiency in Microsoft Office and general computer skills.
- Excellent multitasking and problem-solving abilities.
- A compassionate, patient-centered approach to care.
- Bilingual in Spanish is essential for effective communication.
