Job Requirements
The Physical Therapist (PT, DPT, RPT) in Home Health is responsible for:
Evaluating patient care needs related to functional status/mobility, endurance/stamina, pain, and other medical conditions.
Developing and implementing treatment plans to improve and enhance patient well-being.
Evaluating patient progress and reporting observations and changes in condition to the supervising nurse and/or physician.
Treating patients to relieve pain, develop and restore function, and maintain maximum functioning using accepted standards of practice.
Instructing and informing healthcare team members, patients, and family/caregivers regarding the plan of care, functional limitations, and progress toward goals.
Licensure Requirements
- Current Physical Therapy licensure in the state of employment
- Current CPR certification
- A current driver's license and vehicle insurance, with access to a dependable vehicle
