Position Title: Empowerment Coach/ Case Manager
Classification: Regular Full-time
RAPID RE-HOUSING (RRH) provides rental assistance and supportive services for up to 12-months. The Program goals are to help people obtain housing quickly, increase self-sufficiency, and stay housed. It is offered without preconditions (such as employment, income, absence of criminal record, or sobriety) and the resources and services provided are typically tailored to the needs of the person. Rapid re-housing is an intervention designed to help individuals and families to quickly exit homelessness, return to housing in the community, and not become homeless again in the near term. The core components of a rapid re-housing program are housing identification, move-in and rental assistance, and case management and services.
The Empowerment Coach is responsible for providing ongoing case management services for individuals and families experiencing homelessness who are rehoused in apartments throughout Dallas County. The primary goal is to address the barriers that impact maintaining permanent housing using a strengths-based, Housing First approach. Functions performed include housing location and navigation, rental assistance and rental payment monitoring, connection to community resources, home visits, goal planning for housing stability, financial education, employment and coaching, landlord engagement, and problem solving with other Case Managers. Work is accomplished by advising and motivating program participants to accomplish goals in their Housing Stability Plan.
ESSENTIAL TASK:
- Take full responsibility and ownership for job description
- Must be able to facilitate tailored case management to address the unique needs of each household served utilizing Housing First, Low Barrier, Harm Reduction, Trauma Informed, and Person-Centered Approach
- Employ motivational interviewing to assist and support participants in addressing permanent housing barriers
- Promote housing stability with budgeting, self-care, living skills, and developing community supports
- Develop and update Housing Stability Plans with participants, to prioritize short and long-range goals
- Travel to different apartment communities throughout Dallas County and perform monthly home visits (paused)
- Work with the participant to resolve issues with the landlord, as needed
- Work collaboratively as a team to advance the housing goals of the participant
- Maintain accurate and timely records in the Homeless Management Information System (HMIS) database and other case files, documenting interactions with participants in the standardized format with HIPPA guidance
- Capture weekly and quarterly measured outcomes and other required outcomes as directed
- Maintain effective working relationships with team members and other community providers
- Participate in HCC special events, attend staff meetings and other required workshops or meetings
- Other duties as required
EXPERIENCE, KNOWLEDGE, AND SKILLS:
- Understanding of the disparity of homelessness as it relates to long-standing historical and structural racism
- Experience working with individuals with undiagnosed mental illness
- Knowledgeable about the principles of de-escalation
- Teachable team-player with a high confidence level in working independently
- Adept at assessing and developing rapport with others
- Knowledge of community resources available to households
- Ability to be flexible, adapt and adjust to rapid change within a fast-paced and growth-oriented environment
- Possess and maintain a positive, solution-oriented approach
- Good time-management skills, well-organized with the ability to prioritize, and shift priorities, as needed
- Strong verbal, written, and active listening skills
- Proficient in MS Office software (Word, Excel, PowerPoint)
- Ability to write standard case notes that are Clear, Concise, Relevant, and Useful
- Ability to apply situational reasoning, exercise judgment, decisiveness in situations involving conflicts
- Ability to add, subtract, measure, multiply and divide, and calculate percentages, fractions, and decimals
QUALIFICATIONS
- Bachelor's degree in a Human Services field with a minimum of two-years’ experience
- Valid Texas driver's license and auto insurance
SALARY AND BENEFITS $44k - $46k, commensurate with work experience
