Housing El Dorado is a dynamic young non-profit on the Western Slope of El Dorado County committed to increasing affordable housing and services for our homeless neighbors.

The Case Manager is responsible for providing case management to persons experiencing homelessness and residing in a place not meant for habitation. The Case Manager will develop individual plans to transition people out of homelessness and into stable housing; establish relationships with multiple community agencies for supportive services; assist people with public benefits, employment, education access, income support; crisis intervention; and provide resources and referrals as needed. 

 We need a self-starter who is flexible to meet a variety of needs and will work to prioritize and complete the following kinds of tasks:

  • Maintain positive relationships with program participants, landlords, HED staff, and all community service agencies. 
  • Assess clients needs and develop an individualized plan to develop self sufficiency and obtain housing.
  • Encourage clients to proactively take steps to make progress on their plan.
  • Meet regularly with clients to assess progress and identify next steps and reassess needs and barriers.
  • Maintain a thorough case file on each client
  • Coach, support and guide clients through the steps in their plan; assist them with fears and challenges they face.
  • Encourage Clients to proactively search for housing
  • Help Clients to understand the relevant options in their housing search
  • Conduct or facilitate site visits to confirm that a Client’s intended home meets relevant guidelines for habitability
  • Review the terms of leases to ensure that they meet relevant requirements and that they will be sustainable
  • Assist Clients with the logistics, equipment, furniture, and emotional support needed for successfully moving into a new home
  • Work with Clients to resolve any conflicts, difficulties, or problems that arise after MI
  • Promote Clients’ long-term housing stability by helping them to make progress on life goals such as: employment, family reunification, mainstream benefits, etc
  • Regularly communicate with Navigators, Coordinated Entry (CE), and other staff regarding housing availability and participant housing needs. Visit each designated community location at least monthly.
  • Collect and report program data, including, but not limited to, tracking housing placements and case notes in HMIS and other required data
Qualifications & Experience
  • BA degree in social work or related field; equivalent experience in human service field will be considered.
  • Excellent verbal and written communication, including cultural competency skills
  • Demonstrated ability to work with diverse populations, including persons experiencing homelessness
  • Strong documentation skills, including maintenance of paper files and online data
  • Familiarity with and commitment to the principles and practices of Housing First, trauma-informed and participant-centered care, harm reduction, motivational interviewing, and safeguarding participant confidentiality.
  • Experience working on a team and being collaborative
  • Knowledge of community resources for households with very low income

Salary & Benefits:
$22-26/hour for 30-40 hours per week
Sick leave and paid vacation time
