Accordingly, the RWDSU has the exclusive right to bargain over the terms and conditions of employment related to this position and this position may become covered by the terms of a collective bargaining agreement (a “CBA”) between Housing Works and the RWDSU
Compensation Range: $18/hr - $21/hr commensurate with experience
Overview:
The M-PACT Prevention Navigator (PN) will implement targeted HIV prevention strategies for New York City’s most vulnerable Queer Black and Brown community members, with a focus on Black and Latino men, as part of the Men’s Prevention, Action, Care, and Treatment (M-PACT) Program. The primary responsibility of the PN is to provide culturally appropriate testing, education, facilitation of evidence-based intervention groups, and linkage services. This includes conducting Fourth Generation HIV Testing, Hepatitis C (Hep C) Testing, community-based STI testing, and utilizing innovative tools like Binx io Point-of-Care Testing.
The PN will also assess and address Social Determinants of Health (SDOH) impacting employment, education, housing, and healthcare access, providing advocacy and support to minimize barriers. Through both in-person and online outreach initiatives and participation in community events, the PN will engage Queer Black and Brown community members in culturally relevant ways, building trust and increasing access to HIV, Hep C, and STI prevention resources. This role is essential for achieving program goals and promoting equitable access to prevention and care services for underserved communities.
Responsibilities:
- Participate in outreach initiatives targeting the key population for rapid HIV testing and general engagement.
- Engage with Black and Latino men in culturally relevant ways to build trust and increase program participation.
- Ensure at-risk and newly diagnosed clients are linked to primary care for confirmatory testing, STI screenings, and PrEP/PEP services when applicable.
- Work collaboratively with peers, outreach staff, and care managers to improve access to prevention services and retention in care.
- Conduct follow-up outreach with linked clients via various methods (in-person, phone, social media) to encourage consistent engagement in services, including primary care, mental health, and substance use treatment.
- Conduct Brief Interventions, Intake Assessments, and navigation activities.
- Participate in integrated case conferences to enhance care coordination.
- Attend community events, such as health fairs, LGBTQ events, and testing events, to engage new clients for prevention services.
- Participate in Housing Works advocacy events (minimum of four per year, July–June).
- Document contract notes and data entry in AIRS, eCW, and e-ICare systems and maintain weekly tracking forms to record outreach and engagement efforts.
- Participate in weekly supervision.
- Engage in department activities, including new staff onboarding, YPS Pride Committee, and Continuous Quality Improvement (CQI) efforts.
- Attend recurring interagency and community meetings to improve integrated care and maintain partnerships.
Job Requirements
- Required: High School Diploma or GED
- Preferred: Associate’s or Bachelor’s degree (2- or 4-year) in Public Health, Social Work, or a related field.
