Overview: This position provides support to, case coordination for, and primary care services to clients who lack access to healthcare.
Duties and Responsibilities:
Communicates with others (internally and externally) to provide, exchange, or verify information, answer questions, and address issues of clients
Conducts Brief Assessments to ensure patients are adequately screened for level of case management care
Answers and returns patient telephone calls, schedules and executes reminder calls for Infectious Disease, ID, service line appointments
Provide outreach opportunities for clients who lack access to healthcare and provide enrollment and application follow-up assistance
Participates in multidisciplinary case staffing in order to ensure coordination of care and high-level of services provided to patients of the health center
Familiarity with HIV/AIDS, substance abuse services and treatment, homeless services, women’s health and other community services
Objectivity and self-discipline to avoid emotional involvement in situations that are frequently emotional and high charged
Keeping abreast of new knowledge and techniques related to the practice of case management and new medical treatment modalities as they might affect the social adjustment or lifestyle of clients via literature, professional settings and staff development activities
Documentation of services provided in client record and required databases(s) including data entry in EMR, CPCDMS, GPRDA, ARIES, etc.
Perform quality management/assurance activities
Serves as an ADAP Enrollment Worker backup in order to ensure continuation of patient medication access/coverage
Conduct the 6-month attestations for all enrolled clients in accordance with THMP policies. Obtain, maintain, and submit to THMP all updated eligibility documentation
Conduct annual Re-Certifications for enrolled clients in accordance with THMP policies. Obtain, maintain, and submit to THMP all updated eligibility documentation
Obtain, maintain, and submit the required documentation for client application including residency, income, and the THMP Medical Certification Form (MCF)
Explains ADAP program benefits and requirements; and assist clients with the submission of complete, accurate APAP applications
Documents all ADAP interactions ending in submission in both EMR and ARIES
Attend all ADAP trainings
Other duties as assigned
Education, Experience, Licensure/Certification and Skills/Abilities Related Requirements:
Bachelors Degree; Social Work, Social Services or related major preferred;
At least one (1) year of practical work experience in provision of services 10 PI-WHA or RWGA waiver
Must demonstrate case management skills and knowledge of associated documentation
Must meet Standards of Care as prescribed by the respective funding source
Bilingual in English and Spanish preferred
Continuing Education and Training Requirements:
Participates in training required by the funding source and/or required by licensing board.
