What will I bedoing?
This position provides care management services for older persons and their family caregivers living in a 12-county region, surrounding Harris County. Care management activities include, however not limited to :
ü Conduct client interviews and needs assessments.
ü Work with client to address needs.
ü Make service delivery recommendations.
ü Coordinate client services with qualified service providers.
ü Perform client follow-up calls.
ü Enter client case notes and service deliver in the State's information management system.
ü Provide monthly reporting.
ü Program Coordinator works under limited supervision with considerable latitude and uses independent judgment.
Position requires working in agency's southwest satellite office located in Richmond, Texas.
KeyQualifications
ü Experience Case Manager who is knowledgeable of human services programs, preferred
ü A passion for working with older individuals and their family caregivers.
ü Ability to multi-task and handle a high volume of work.
ü Must show attention to detail and the ability to competently complete work tasks.
ü Ability to resolve customer issues and making recommendations.
ü Proficiency in Microsoft Suite.
