Qualifications:
- Graduate of an accredited L.V.N. school or through reciprocity.
- Currently licensed as an L.V.N. in the State of Texas.
- One year experience as an L.V.N.
- Skilled in nursing practices with Home health, hospice, or oncology experience preferred.
- Current certification in basic life support.
- Ability to cope with family emotional stress and tolerant of individual lifestyles.
- Good organizational skills are required to handle a variety of tasks simultaneously.
- Good communication skills, both oral and written.
- Effective interpersonal skills.
- Ability to accommodate multi-faith beliefs and backgrounds of patients/families.
- Ability to function as part of an interdisciplinary team.
- Must have reliable transportation to work assignments.
- Knowledge/proficiency in applicable federal/state laws governing hospice licensure is preferred.
Duties/Responsibilities:
- Provide care for patients and families according to an individualized plan of care and by agency policies and regulations that define the scope of practice.
- Participate in shift assessments, administration of medications, and bedside care.
- Assist patient/family in learning appropriate care techniques.
- Assist and support the R.N. and/or physician in performing specialized duties related to end-of-life care.
- Report symptoms and changes in condition to the Charge Nurse, and physician.
- Maintain accurate, up-to-date, and timely documentation of nursing care provided to all patients.
- Ensure problems, actions, specific outcomes, and responses to care are clearly stated and readily available for review.
- Rotate weekend/holiday duty as assigned.
- Participate in QA/PI activities to ensure continuous quality assurance and process improvement.
- Assist in providing services that foster the organization’s mission, values,s and goals.
- Responsible for timely completion of paperwork required to meet State and Federal regulations. Documentation should be accurate and detailed to meet these regulations.
- Understand and adhere to organizational compliance and privacy programs.
- Assists with the orientation of new staff.
- Performs other job-related duties as required.
