At Houston Hospice, we focus on caring, not curing. We take a team approach to care that includes physicians, nurses, social workers, chaplains, hospice aides, volunteers, and bereavement counselors. We provide hospice service wherever patients and families need us: in their home, in a long-term care facility, or in our inpatient care unit that is based in the Texas Medical Center. Our team of doctors and nurses specialize in managing pain and symptoms that are unique to life-limiting illnesses.
We are seeking a candidate who would be responsible for the overall management of a patient's case. Responsibilities include coordination of care with the client, family, and other providers.
Duties and Responsibilities:
- Coordinate care with providers, including: Registered Nurses (RN), Licensed Practical Nurses (LPN), and Licensed Practical Nurses (LPN/LVN)
- Maintain case documentation in accordance with state regulations.
- Maintain case notes in accordance with state regulations.
- Develop and maintain care plans for each client on the basis of clinical assessment and family needs.
- Coordinate care with other providers as necessary
- Maintain appropriate communication with the admitting physician or psychiatrist regarding progress reports, changes in status, and as necessary, refer patients to other providers or services.
- Conduct home visits as necessary to ensure optimal outcomes.
- Review patient charts weekly to ensure appropriate follow-up and referral as necessary.
- Ensure that appropriate follow-up is provided by all other providers as necessary.
- Coordinate care with community agencies as necessary to ensure appropriate follow-up.
- Provide training to staff on client management and referral processes.
- Develop, modify, and maintain case files as needed in accordance with state regulations.
- Adhere to all policies and procedures of the agency or organization that employs you.
