At Houston Hospice, we focus on caring, not curing. We take a team approach to care that includes physicians, nurses, social workers, chaplains, hospice aides, volunteers, and bereavement counselors. We provide hospice service wherever patients and families need us: in their home, in a long-term care facility, or in our inpatient care unit that is based in the Texas Medical Center. Our team of doctors and nurses specialize in managing pain and symptoms that are unique to life-limiting illnesses.
Summary:
We are seeking a candidate for our Pediatric Butterfly Team who would be responsible for the overall management of our pedi. cases. Ideal candidate would be responsible for the coordination of care with the client, family, and other providers.
Duties and Responsibilities:
- Perform direct patient care based on the nursing process and within the scope of the Registered Nurse Practice Act, including identifying nursing problems, psychosocial, spiritual and emotional issues as they relate to the individual needs of the patient and family.
- Assist with pain and symptom control related to patients.
- Assist patient/family with acceptance, cooperation and response to the hospice concept.
- Prepare a weekly visit schedule and submit to PCM.
- Willingness and ability to treat adult hospice patients as well as pediatric hospice patients.
- Must be willing and able to carry a minimum patient care load of 15 patient visits weekly. This can be a combination of both pediatric and adult patients.
- Participate and contribute to IDT meetings for both pediatric and adult patients.
- Report any significant changes in patient’s condition to the attending Physician and/or Patient Care Manager and request change orders as needed.
- Instruct clients on appropriate techniques for care of the patient and documents following established Plan of Care.
- Understands and adheres to organizational compliance and privacy programs.
- Assists in providing service that fosters the organization’s mission, values, vision and goals.
