 Role Overview:

We are seeking  compassionate and dedicated Nurse Practitioners / Physician Assistants to join our team. The ideal candidate will provide high-quality primary and specialty care, with a focus on LGBTQ health. This role requires a commitment to patient-centered care, cultural competency, and a collaborative approach to healthcare delivery.

Key Responsibilities:

  • Conducts comprehensive patient assessments, including health history, physical examinations, and diagnostic tests.
  • Develops and implement individualized treatment plans based on patient needs and clinical guidelines.
  • Provides preventive care, health education, and counseling to patients.
  • Manages chronic conditions, including but not limited to, HIV/AIDS, diabetes, and hypertension.
  • Collaborates with interdisciplinary team members, including physicians, nurses, social workers, and other healthcare professionals.
  • Maintains accurate and timely documentation in electronic health records.
  • Participates in quality improvement initiatives and continuing education programs.

Qualifications:

- Master's or Doctorate degree in Nursing or Physician Assistant from an accredited program.

- Current Nurse Practitioner or Physician Assistant license and certification in the state of Illinois

- Prior experience in a clinical setting, preferably in primary care or LGBTQ health.

- Strong knowledge of LGBTQ health issues and a commitment to inclusive care.

- Excellent communication, interpersonal, and organizational skills.

- Ability to work effectively in a fast-paced and dynamic environment.

Preferred Qualifications:

  • Experience in community health settings.
  • Knowledge of public health principles and practices.
  • Bilingual in English and Spanish
