What are the responsibilities and job description for the LifeSet Specialist position at Hoyleton Youth and Family Services?
POSITION SUMMARY
The LifeSet Specialist is responsible for providing services to address the needs of young people
17.5-21 who are transitioning out of care and need individualized skill development and support to
transition successfully into adulthood, specifically obtaining and maintaining safe and suitable
housing, continuing their education, learning independent living skills, obtaining and maintaining
employment, building a support system, and remaining free from legal involvement. Works to
establish/re-establish meaningful relationships with parents, siblings or other significant adults; as
appropriate, coordinates family contact. Assures that all service requirements of the DCFS are
complied with, including service plans, sibling visitation and court hearings; manages all aspects of
discharge planning in coordination with the rest of the Team.
ESSENTIAL FUNCTIONS AND RESPONSIBILITIES
- The LifeSet Specialist provides individual services to 6-8 young adults in the home andcommunity setting to assist the young adult with transition to adulthood, meeting a minimum ofone time per week face to face with the young person and providing additional support viaphone, text, or in person as needed. When indicated, a LifeSet Specialist may see a youngperson daily to ensure safety and stability.
2. Additionally, the LifeSet Specialist is directly responsible for all interventions associated with a specific young adult and their family. This includes case management and collaboration with external key stakeholders and referral sources, such as probation, child welfare workers, therapists, etc.
3. The specialist conducts ongoing assessment of the young person’s needs from a strength focused, solution-based perspective and develops service plans on a monthly basis.
4. The specialist also documents all service contacts with the young adult within the timelinesrequired by the organization
5. The specialist is expected to attend all meetings related to the young adult and/or family withthe purpose of gathering information specific to intervention development and advocating inthe best interest of the young adult’s long term stability. Hoyleton offers 24/7 on call to meet ayoung person’s needs after hours and on weekends and holidays. The specialist is included inan on call rotation based on current program expectations.
6. In addition, the specialist is expected to attend and actively participate in individual and teamsupervision sessions and trainings and to follow up and/or implement feedback andrecommendations received from the team supervisor, program director, and clinical consultant.
7. The specialist is expected to adhere to all ethical and legal guidelines, including confidentiality and mandated reporting.
8. Develops service plans as required by the Department of Children and Family Services; attends Administrative Case Reviews, court hearings, IEP meetings and other reviews and staffings, as required.
9. Conducts detailed review of youth’s case record to identify parents, siblings, foster parents and other significant adults from the child’s present and past. Conducts assessments of their capacity and willingness to become or remain a resource to the youth. Establishes a visiting plan and coordinates its implementation.
10.Completes and maintains case records in accordance with DCFS requirements, including faceto-face contact, phone contact, and collateral contacts.
Requirements:SKILLS and ABILITIES
- Knowledge and/or experience working with adolescents preferred.
- Good oral and written communication skills.
- Possess a driver's license in good standing, provide proof of insurance, and a reliable mean of transportation.
- Strong organizational skills and attention to detail.
- Ability to maintain a flexible schedule, and to manage multiple priorities simultaneously.
- Excellent and verbal skills
EDUCATION AND/OR EXPERIENCE
- Master’s degree in field of social work, psychology, or related field preferred. Bachelor’s degree in social services with two years related experience in counseling and/or case management is required.
- Licensure by DCFS or eligible for licensing.
