Description

POSITION SUMMARY

Responsible for providing direct and indirect medically necessary rehabilitative services such as assessment and therapy in compliance with individual treatment plans and behavior intervention plans, under the direction of the of Assistant Residential Director of Clinical Services .

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES

  • Participates in the intake process by reviewing referral packets in conjunction with the Clinical Services Supervisor, attending the admission meeting, and contributing to the assessment of the appropriateness of the youth for placement.
  • Serves as clinical leader to other colleagues, works collaboratively with the Program Manager and other cottage staff to ensure a consistent, positive, strengths-based therapeutic environment and to encourage staff to maintain healthy attitudes and boundaries and fully participate in the treatment process.
  • Provides diagnostic assessments and develops the Illinois Medicaid Comprehensive Assessment of Needs and Strengths (IM CANS) and develops an Individualized Crisis Management Plan (ICMP). Reviews/revises the IM CANS and ICMP a minimum of every three months.
  • Assess for the need of additional plans: Supervision Plan, Risk Plan, Right’s Restoration Plan. Develop and review/revises the plans a minimum of every three months.
  • Provides individual, group and family therapy to client as prescribed in the IM CANS. Provides crisis intervention as necessary. Works directly with identified client and their identified support system to help them adjust to their life situation and to plan for their future needs.
  • Participates in transition planning for residents to help facilitate their subsequent placement/future needs.
  • Participates in Utilization Reviews as requested.
  • Collaborates with external agencies and programs, and with families to coordinate services on behalf of the individuals served.
  • Assumes after-hours/weekend on-call responsibility on a rotating basis, as assigned, for a minimum of one (1) week at a time.
  • Consistently completes all required documentation in accordance with established agency timelines and expectations.
  • Is prepared for staffing's and lead the clinical portion of the staffing, provides a minimum of one hour of structured clinical supervision to cottage staff per month, attends team meetings (i.e. “Rounds”, etc) as scheduled, and actively participates in individual supervision with the Clinical Services Manager.
  • Assist with case management responsibilities when needed.
  • Represents agency in professional organizations by attending meetings as directed.
  • This Position requires on call status whenever applicable.
  • Other duties as assigned.

SKILLS And ABILITIES

  • The ability and willingness to actively participate in a fast-paced, multidisciplinary team environment.
  • Knowledge of evidence-based practices and a variety of treatment modalities used with the population(s) served. Knowledge of the developmentally disabled population and a creative approach to providing therapeutic services.
  • Must possess the experience, personal qualities and practice skills to work effectively with professionals in other organizations and from other disciplines and to provide services in a manner that reflects understanding and appreciation of multiculturalism.
  • Strong organizational and time management skills.
  • Excellent interpersonal and written communication skills.

EDUCATION AND/OR EXPERIENCE

  • Master’s Degree in a license-eligible human services field required; strong preference of Illinois license in Clinical Social Work or Clinical Professional Counseling
  • Minimum of two years professional experience in a similar setting preferred.

REQUIREMENTS

  • Must be at least 21 years of age
  • Must possess and maintain a valid, Illinois driver’s license
  • Must possess and maintain proof of automobile insurance

CARE & TCI

One of the essential job functions of any position within the agency is to align your performance with our overarching philosophy of CARE. All decisions, actions, communication, and performance should align with the six CARE principles (Developmentally Focused, Family Involved, Relationship Based, Ecologically Oriented, Competence Centered, and Trauma Informed). Not only must you complete the CARE training with satisfactory participation, but you must then put that training into practice in your everyday work.

Every employee must also pass TCI training with a satisfactory grade in written and/or physical testing. All employees must remain current on refresher training that is provided monthly at all locations. You will be tested every 6 months on the physical portion, if applicable to your position. You will be tested annually on the written portion regardless of your position. If you are not current on your TCI refresher training every six months, you will be required to retake the full course. If you do not pass either test, you may be required to retake the full course. Not only must you complete the TCI training with satisfactory participation, but you must then put that training into practice in your everyday work.

WORKING CONDITIONS

Work approximately 40 hours per week.

Frequency Options: Occasional: 0 – 15% of time; Frequent: 16 – 50% of time; Constant: Over 51% of time

Occasional – Exposure to noise, Exposure to heights, Exposure to temperature above 80 degrees, Exposure to chemicals, Exposure to hazards, Exposure to dust, Carrying approximately 10 feet, Pushing approximately 5 pounds, Lifting approximately 5 pounds, Climbing approximately 15 – 20 steps, Bending, Squatting, Kneeling and Physical Restraints.

Frequent – Walking approximately 100 feet and Standing

Constant – Working alone or independently

$55,265.60- $82,898.40 yearly
