Description : POSITION SUMMARY
The Wraparound Service Coordinator works in collaboration with the youth, family, guardian, caseworker, and DCFS. Responsible for the design and delivery of services that meet the youth's needs and provide support to youth and families who have been referred to the Wraparound Program.
ESSENTIAL FUNCTIONS AND RESPONSIBILITIES
- Evaluate and interpret the referral information. Complete a strength-based Discovery Assessment with the child and family and the Child and Adolescent Needs and Strength Assessment (CANS) at the time of admission and discharge.
- Develop, document, and maintain a Crisis Plan and individualized Wraparound Plan in conjunction with the youth, parent, guardian, caregiver, and caseworker of referring agency.
- Conduct weekly face-to-face home / community visits with the assigned youth and their families / support system. Participate in monthly Child and Family Team Meetings for each assigned youth to review care plans and progress on needs. Maintain meeting minutes for CFTM as needed.
- Using resources and available flex funding to assure that services are based specifically on the needs of the child and family and are authorized by the Wraparound Plan and are reimbursed in a timely manner.
- Deliver strength-based, family-centered, culturally competent, and trauma-informed facilitation services.
- Engage with treatment providers, schools, and other collateral contacts to ensure youth's needs are being met.
- Oversee transition to any ongoing care after discharge from Wraparound Program.
- Maintains case records and notes in a timely and professional manner; in accordance with the Wraparound Program Contract.
- Completes other duties as assigned.

CARE & TCI
One of the essential job functions of any position within the agency is to align your performance with our overarching philosophy of CARE. All decisions, actions, communication, and performance should align with the six CARE principles (Developmentally Focused, Family Involved, Relationship-Based, Ecologically Oriented, Competence Centered, and Trauma-Informed). Not only must you complete the CARE training with satisfactory participation, but you must then put that training into practice in your everyday work.
Every employee must also pass TCI training with a satisfactory grade in written and / or physical testing. All employees must remain current on refresher training that is provided monthly at all locations. You will be tested every 6 months on the physical portion, if applicable to your position. You will be tested annually on the written portion regardless of your position. If you are not current on your TCI refresher training every six months, you will be required to retake the full course. If you do not pass either test, you may be required to retake the full course. Not only must you complete the TCI training with satisfactory participation, but you must then put that training into practice in your everyday work.
