What are the responsibilities and job description for the Case Manager position at HP Recruitment?
As an Enrollment Case Manager, you will travel throughout the assigned county working in applicants’ places of residents. Conducting intake visits for waiver applicants and acts for the eligibility determination.
Summary of Qualifications:
- Traveling throughout assigned county working conducting intake visits for waiver applicants and facilitating the eligibility determination process
- Providing assistance from the time individuals are identified as a potential participant in a program to the time they are in rolled as participants
- Review and screen the provider application and relevant documentation accompanying the application
- Initiate the development of individual service plan and obtain his department approval
- Assist with developing relationships with community-based organizations, advocates and stakeholders to gather feedback on improving the application process and removing barriers to enrolling and waiver programs
- Contact providers to obtain missing or incomplete information from the provider application
- Work with Site Visit Field Staff to collect and verify results from site visit
Education and Experience:
- A minimum of one year experience in public or private social work and a Bachelor’s Degree
- Two years of case work experience including one year of experience performing assessments of client’s functional ability to determine the need for institutional or community-based services

Pay: $20.00 per hour
