SUMMARY OF POSITION:

The After Hours Nurse provides skilled nursing care to individuals and families, based on an ongoing nursing assessment of need. He/She plans, implements, and evaluates nursing care through systematic assessment of health needs and optimum utilization of personnel. Provides care and assistance to patients and families during on-call hours.

ESSENTIAL FUNCTIONS:

Requires full range of body motion including handling, lifting of patients, manual finger dexterity and eye-hand coordination. May require standing and walking for extensive periods of time. Must be able to lift at least 50 pounds using proper body mechanics. Requires corrected vision and hearing or normal range. Requires working under stressful conditions or working irregular hours.Frequent exposure to communicable diseases, toxic substances, medicine preparations, bodily fluids and conditions common in a terminally ill patient environment. Must be able to arrange travel and be able to access client homes independently.

DUTIES:

1. Assumes primary responsibility for a patient/family caseload that includes the

assessing, planning, implementing and evaluation phases of the nursing process.

2. Initiates communication with attending physicians, hospice staff

members and other agencies as needed to coordinate optimal care for the patient/family.

3. Maintains regular communication with Clinical Team Coordinator and patients

case manager.

4. Maintains regular communication with the attending physician concerning

patient/family.

5. Obtains data on physical, psychological, social, and spiritual factors that may

influence patient/family health status and incorporate data into the plan of care.

6. Maintains up-to-date patient records so that problems, interventions, and goals

are accurately and clearly stated and changes are reflected as they occur.

8. Accepts responsibility for coordinating physical care of the patient by teaching

primary caregivers, volunteers, employed caregivers, or by providing direct care

as appropriate.

9. Meets regularly with hospice staff to review problems or unique issues from

caseloads, shares professional support, and exchanges feedback aimed toward

enhancing professional growth.

10. Informs the primary care nurse and/or the Clinical Team Coordinator of unusual or potentially problematic patient/family issues.

11. Assumes role of primary nurse when responding to patients/families of other

staff nurses during on-call time, or as requested by the Clinical Team Coordinator to fill patient/family needs during the absence of other staff nurses.

12. Inputs documentation into the computer software system within the

requirements set forth by the agency.

13. Provides appropriate support at time of death and period of bereavement.

14. Participates in mandated orientation and in-service programs for professional staff.

QUALIFICATIONS:

Professional:

- Graduate of an accredited School of Nursing.

- Current licensure in the state of Kentucky.

- Knowledge of current trends and techniques in nursing practice.

- At least one year experience in a patient care position in a hospital, nursing home, or related healthcare facility preferred.

- Effective documentation skills.

- Demonstrates effective verbal and written communication skills.

- Desires to work as part of a team and willing to represent the principles, views and mission of Hospice and Palliative Care of Western Kentucky.

Job Type: Part-time

Pay: $32.00 - $35.00 per hour
