Overview:
We are seeking a dedicated Social Worker to join our team. The Social Worker will be responsible for providing compassionate and comprehensive social work services to individuals in need. Working collaboratively with a multidisciplinary team, the Social Worker will assess, plan, and implement interventions to support clients in various settings.
QUALIFICATIONS:
Professional:
- Master’s of Social Work Degree required
- Must be a Licensed Social Worker.
- At least one year experience in healthcare setting.
- Knowledge of current medical terminology.
- Ability to comprehend applicable federal, state and local regulations and policies, procedures and programs.
- Hospice experience with knowledge of social services regulations preferred.
- Desires to work as part of a team and willing to promote the principles, views and mission of Hospice and Palliative Care of Western Kentucky
Duties:
- Conduct assessments to identify clients' needs and develop individualized care plans.
- Provide counseling and support to individuals dealing with addiction, mental health issues, hospice care, and behavioral health concerns.
- Collaborate with healthcare professionals to ensure holistic care for clients.
- Assist clients in navigating healthcare systems and accessing community resources.
- Document client interactions and progress accurately in electronic records.
- Advocate for clients' rights and empower them to achieve their goals.
- Participate in case conferences and team meetings to discuss client progress and treatment plans.
Experience:
Required Skills:
- Experience in addiction counseling, mental health counseling, hospice care, and behavioral health.
- Familiarity with medical terminology.
- Ability to work in outpatient, assisted living, and nursing home settings.
- Strong communication skills, both verbal and written.
- Proficiency in electronic documentation.
- CPR certification.
Job Type: Part-time
Pay: $22.00 - $28.00 per hour
Expected hours: No more than 25 per week
