JOB SUMMARY
HRDI is seeking a compassionate and community-driven Community Health Worker (CHW) to serve as a vital link between patients and the health care system. As a CHW, you will promote health education, provide care navigation, and help individuals gain access to vital resources. This position requires strong ties to the community and a passion for reducing health disparities through outreach, advocacy, and patient support.
JOB DUTIES
- Serve as a liaison between the community and health care/social service systems.
- Help individuals understand their conditions and care plans.
- Deliver culturally appropriate health education and support.
- Connect clients with resources such as food, housing, and health insurance.
- Conduct health screenings (e.g., blood pressure checks).
- Assist with scheduling appointments and completing applications.
- Support clients with chronic disease management and behavior change.
- Participate in community outreach events and conduct group presentations.
- Maintain regular communication via phone, in-person, text, and social media.
- Document client progress and manage data using EHR systems.
- Build and maintain strong working relationships with patients and partners.
QUALIFICATIONS
- Completion of a GED or high school diploma.
- Completion of (CHW II) or enrollment in (CHW I) a CHW certification program.
- Minimum of 2–3 years of experience in customer service, outreach, or other community-facing roles, preferably in healthcare, public health, or social services.Knowledge of local population demographics, assets, and needs.
- Knowledge of local community health disparities.
- Ability to establish patient connection and provide ongoing support in behavior change.
- Demonstrated organizational and time management skills.
- Effective written and oral communication skills to document patient information and progress.
- Computer literate; proficient in Microsoft Word, Excel, and PowerPoint.
PHYSICAL REQUIREMENTS
- Moderate physical activity may include lifting 25–50 lbs.
- May require extended periods of standing or walking.
- Minimal exposure to physical risk.

Occupational Risk for Exposure to TB:
There is an occupational risk for exposure to TB, as this position involves extensive community and patient contact. Employees in this role are required to comply with annual TB testing as part of workplace health and safety protocols. The only acceptable exceptions to this requirement are documented medical contraindications or religious objections. Employees who decline TB testing will be required to wear a mask when deemed necessary.
