Coordinator Home Care Services

Coordinates daily activities in home health and hospice. Supports continuity of care and maintains effective communication between staff and customers. Assists with staffing patterns, communication, supply management, patient quality and patient safety. Ensures that care and service are delivered and documented appropriately.

Education Qualifications

• Associates degree in nursing or related clinical field is required.
• Bachelor’s degree in nursing or related clinical field is preferred.

Experience Qualifications

• 3 years of clinical experience is required.
• 1 year of demonstrated leadership experience is preferred.
• Prior experience in home health or hospice is preferred.
• Valid state driver’s license, current automobile insurance and reliable automobile is required.

Certifications, Licenses and Registrations

• Illinois: Registered Nurse (RN), Physical Therapist (PT), or Occupational Therapist (OT) license is required.
  
  Wisconsin: Registered Nurse (RN), Physical Therapist (PT), or Occupational Therapist (OT) license is required. Basic Life Support (BLS) is required. Wisconsin: RNs may practice with a valid multi-state RN license as outlined in the Nurse Licensure Compact rules.
