What you will be doing
RESPONSIBILITIES
- Service as a consistent resource for patients and providers to answer questions or concerns
- Monitor and manage patient workqueues, perform check-in and check-out, and ensure encounters are completed and closed
- Directly schedule and facilitate appointments with various physician practices or services both within HSS and externally, taking ownership of the follow-up until completion
- Answer incoming calls from patients who have completed initial enrollment in the program and perform outbound communications for follow-up
- Obtain and upload authorizations and results to the patient's chart for provider follow-up
- Pre-appointment screening to ensure patients are appropriate for the program and should not be either scheduling to a physician practice or seeking emergency care
- Answer patient questions, obtain letters and prescription renewals, and help patients navigate their next steps in the treatment plan as directed by our providers
- Manage timekeeping and assist with provider coverage to ensure program is staffed when possible
- Collaborate and build meaningful relationships with providers, care navigation team, physician practices, and ancillary departments; prioritizing tasks to ensure patients receive timely follow-up
