POSITION SUMMARY:

Huckleberry Youth Programs’ mission is to work in solidarity with young people and their families to advance their personal growth and social equity. We are a community based organization that provides a spectrum of services to youth between the ages of 11-24 and their families in San Francisco and Marin counties.  Our programming focuses on the core areas of safety, health, justice, and education. 

The counseling program at Huckleberry Youth Programs in San Francisco offers individual and family therapy as well as case management, groups and behavioral health classes. We strive to provide culturally relevant, high quality services in English, and Spanish to youth (ages 11-24) and their families.  Therapists are embedded at all of our programs sites in San Francisco including the Huckleberry Youth Health Center, Huckleberry House youth shelter, the Huckleberry Advocacy and Response Team (HART), and the Community Assessment and Referral Center (CARC). Placement at schools or other community sites may occur as part of the positions. The Counseling team meets weekly for group supervision and/or staff meetings. Clinicians are expected to maintain a caseload that enables them to book at least twenty clients per week.

This TAY (Transitional Age Youth) Outpatient Psychotherapist/Clinical Social Worker position will be embedded at the Huckleberry Youth Health Center in San Francisco located at 555 Cole Street. The primary responsibility of this position will be providing individual therapy and linkage to services for TAY clients (ages 18-24) who are referred through Coordinated Entry for Youth Access Points and other programs serving youth experiencing homelessness. The embedded Therapist works in collaboration with: the Client Services team (Case Management Therapy), the Create 2 Activate Project Coordinator, the Health Education team and the Cole St Clinic which is run in partnership with the Department of Public Health. The combined teams together provide free and confidential wraparound services for 12-24 year olds. In addition, this site provides basic needs support, Narcan and other Harm Reduction services. 

 

ESSENTIAL FUNCTIONS:

  1. Provide individual therapy to TAY clients (ages 18-24) who are referred through Coordinated Entry Youth Access Points and other programs serving youth experiencing homelessness. 
  2. At least once quarterly provide outreach, educational, and/or therapeutic services to youth at housing sites in the community. Additionally, outreaching to Coordinated Entry and Housing staff at least once quarterly for referral purposes.
  3. Conduct assessments with youth, including the CANS, the TESI and the Huckleberry Youth Health Center Assessment for Services.
  4. Ensure youth are linked to additional supportive services as appropriate.  This may include psychiatric care, substance use disorder treatment, case management, and primary medical care.
  5. Ensure youth feedback is obtained by utilizing the PCOMS (Partners for Change Outcome Rating Scale) at the end of each session, and giving youth surveys (both SF County BHS and Huckleberry Youth Health Center specific).
  6. Attend monthly Health Center All Site and bi-weekly Huckleberry Youth Health Center team meetings.  Attend weekly supervision sessions with the Clinical Director, biweekly check-ins with site Director, embedded therapist meeting, group supervision sessions and agency wide meetings.
  7. Conduct training for Huckleberry staff as needed.  Provide consultation and/or training to Coordinated Entry or Housing site staff as requested.
  8. As part of the Huckleberry Youth Health Center team, assist youth in checking into the Health Center by answering phones, logging youth in, and assisting youth in the navigation of services.
  9. Seek to reduce barriers to accessing mental health services, by demonstrating flexibility in approach and location of services.
  10. Demonstrate appropriate knowledge and skill in conducting biopsychosocial assessments, evaluating and responding to clinical risks, creating diagnostic formulations, developing treatment plans and choosing and applying effective interventions to bring about therapeutic change.
  11. Develop rapport with youth in a manner that demonstrates awareness of the impact of sociocultural factors on experiences, values, learning and counseling needs.
  12. Demonstrate ability to engage and retain youth in services.
  13. Demonstrate thorough knowledge of legal and ethical issues and apply this vigilantly to clinical practice. Consistently maintain appropriate boundaries with clients and staff.
  14. Perform accurate, thorough and timely record-keeping and documentation.
  15. Consistently work to advance clinical skills and knowledge of self and the clinical team by actively engaging in group and individual supervision, pursuing training opportunities and sharing knowledge.
  16. Facilitate behavioral health groups or presentations as directed

WORK ENVIRONMENT:

This position operates out of the Huckleberry Youth Health Center and may include travel to other Huckleberry sites or community locations for service delivery and coordination. This role routinely includes the use of standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines.The Huckleberry Youth Health Center is located in San Francisco.

 

PHYSICAL DEMANDS:

The employee must be able to lift and/or carry up to 10 pounds unassisted on an occasional basis. While performing the duties of this job, the employee is regularly required to talk, hear and sit. The employee is required to stand, walk, climb stairs and use hands to find, handle, or feel objects, tools or controls. 

Qualifications


REQUIRED EDUCATION AND EXPERIENCE:

1. A master’s degree in social work or counseling

2. A clinical license or registered with BBS in pursuit of license 

PREFERRED EDUCATION AND EXPERIENCE:

1. Experience providing services to adolescents or TAY

2.          Experience providing individual therapy

3.      Ability to provide services in Spanish, Cantonese or other San Francisco  Threshold        languages is strongly preferred 

4. Experience with Medi-Cal documentation, especially use of EPIC

5. Experience, awareness and knowledge of cultures of San Francisco youth and families     and demonstration of cultural sensitivity and responsiveness

6. Experience with telehealth and/or other non-traditional modalities.  

ADDITIONAL ELIGIBILITY QUALIFICATIONS:

  1. Recent TB clearance
  2. Valid CA driver’s license and clear DMV record
  3. Have or will complete first aid/cpr training
  4. Fully vaccinated for COVID 19
