What are the responsibilities and job description for the Registered Nurse On Call Coordinator position at HUDSON VALLEY HOSPICE INC?
The After-Hours Coordinator’s (Registered Nurse) objectives include developing strategies to improve service delivered during the AH, weekends, and holidays. This registered nurse position assists the Director of After-Hours Services with departmental oversight, provides direct supervision to their assigned AH staff, and provides clinical and operational support and coordination to staff working during the AH period across all departments and service locations.
ESSENTIAL FUNCTIONS
- Direct supervision of their assigned AH staff, including hiring, orientation, development, performance review, payroll/mileage processing, and scheduling.
- Operational support to staff working during the AH period across all departments with patients across all locations of service and levels of care
- Provides direct patient care when needed to ensure patient needs are met timely and per regulatory requirements.
- Responsible for assuming weekly role as on-call supervisor on a rotating basis.
- Identifies QAPI needs in collaboration with SDAS; and contributes to QAPI data collection.
- Assists SDAS with development and implementation of quality projects to improve patient care, coordination, and service.
- Responsible, in collaboration with SDAS, to review, report, and follow up on all patient/family complaints.
- Reviews and interprets the daily call center on-call log reports to ensure timely documentation of calls, on-call nursing visits, and general performance improvement.
- Responsible in concert with SDAS for the oversight of approval and determination of appropriateness of change in level of care including inpatient admissions, continuous care, respite admissions, re-certification, and revocations from the program, followed by the completion of appropriate documentation.
- Effectively collaborates with all departments and disciplines to coordinate efficient and high-quality hospice services that meet patient needs timely and per regulatory requirements and quality goals (eg. HOPE, visit frequency compliance, CAHPS metrics).

WORK ENVIRONMENT/PHYSICAL DEMANDS

- Flexibility in work schedule
- Moderate degree of possible exposure to highly contaminated agents.
- Regular travel by car in all weather conditions.
- Work in patient homes, hospitals, and nursing homes.
- Moderate to high risk of exposure to unsafe environments
- This position is expected to work evening hours, eg. 2p-11pm with occasional weekend coverage.
PREFERRED EDUCATION & EXPERIENCE
- Bachelor degree in Nursing preferred with three (3) years in which progressive clinical competence and management is demonstrated
- Valid New York State Nursing License
- Valid New York State driver’s license
- Minimum three (3) years of clinical nursing or home-care experience
- Hospice experience preferred
- Leadership experience preferred
- Registered Nurse certified in Hospice and Palliative Care at time of hire or within two years of employment
Hours: Flexible days; evening and weekend hours
Pay: $110,000-115,000 annually
FLSA: Exempt
