Description
The Visit RN is part of the Hospice team to provide nursing services to Hospice patients and their families and to ensure that those services are consistent with the philosophy, goals, objectives and policies of the agency. They work with the case management team to provide coverage in times of increase census, PTO or leaves of absence. The Visit Registered Nurse(RN) is there to support the team and make visits as necessary. This is a part time role with open flexibility in hours and schedule!
ESSENTIAL FUNCTIONS
- Maintain adequate records of all patient/family care services, as defined by policy.
- Assess the needs of terminally ill patients and their families and participates in the development and implementation of the Interdisciplinary Team Plan of Care.
- Utilizes other team members to meet patient needs.
- Revises Plan of Care, as needed, including medications, projected visits, Advanced Directive status, functional limitation, treatments, etc.
- Provides 24 hour continuous skilled nursing care for patients/families in their home, as needed.
- Assists case manager/TL/DPS in the management and support of patient case loads and responds to patient/family needs in accordance with the POC.
- Maintains a working knowledge re: use of CADD-Prizm pump.
- Completes time log accurately and submits with nursing visit report every 3 days.

PREFERRED EDUCATION & EXPERIENCE
- Valid New York State RN License
- Graduate of accredited nursing program
- Minimum of two (2) years paid experience in nursing, experience in oncology, home care or a hospice setting preferred.
- Experience in caring for terminally ill patients and a basic understanding and commitment to the hospice philosophy preferred
- Working in a comprehensive interdisciplinary team approach to care preferred
WORK ENVIRONMENT/PHYSICAL DEMANDS
The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.
- Moderate degree of possible exposure to highly contaminate agents.
- Regular travel by car in all weather conditions.
- Work in patient homes, hospitals and nursing homes.
- Routine duties involve potential for direct exposure to blood or body fluids.
- Ability to carry up to twenty-five pounds of equipment and/or supplies into a patient’s home and transfer/position an average adult for care.
Hours: Varying 20-32 hours /week options - Variable shifts available - open to flexible hours and schedules
Pay: $38-43 per hour
FLSA: Non-Exempt
