The role of this Admissions RN is to establish patient eligibility and appropriate level of care with hospice medical director. Provide physical assessments and coordinate discharge planning as appropriate. Act as Inpatient RN case manager, collaborating with hospital staff for pain and symptom management. Partner in care with physicians, case managers, and other facility staff. Educate patients and families about hospice benefit and services. This role will do both in home and hospital admissions in the Ulster and Dutchess counties.

ESSENTIAL FUNCTIONS :

  • Perform the initial patient / family physical, emotional, psychosocial, cultural, and
  • spiritual assessments to ensure and coordinate the delivery of comprehensive, holistic
  • hospice services.
  • Document all aspects of pre-admission activities, evaluation, and initial assessment
  • thoroughly in the EMR system, ensuring all clinical, medical, emotional, psychosocial,
  • and physical attributes are captured accurately.
  • Follow CMS and agency guidelines to assist the hospice team and Medical Director in
  • determining and supporting clinical eligibility for hospice services and level of care.
  • Perform thorough comprehensive assessments and communicate findings to hospice team
  • members through interpersonal communication and documentation within the EMR.
  • Initiate discussions regarding advanced directives and code status, providing information
  • and answering patient / family questions.
  • Provide anticipatory guidance on disease trajectory and end-of-life symptom
  • management to patients and families to empower them to cope with events as they occur
  • throughout the end-of-life process.
  • Acts as a resource to introduce and arrange for the provision of complimentary therapies
  • as appropriate.
  • Develops and maintains working relationships with hospital staff at each of the contracted facilities to promote and enhance hospice services and continuity of care.
  • Coordinates with hospital and hospice personnel for discharge planning while facilitating a safe plan of care.
  • Communicates effectively with all members of the interdisciplinary team and facilitates discussions surrounding end-of-life, patient status, comfort medications, discharge disposition etc.
  • Develop and prioritize the interdisciplinary care plan goals of the patient / family hospice team and attending physician.
  • Update the care plan a minimum of every 14 days.
  • Provides the necessary education and resource information to hospital staff regarding pain and symptom management in conjunction with the medical director, Director of Patient Access and the VP of Patient Access.
  • Arranges DME delivery and comfort medications upon discharge as applicable.
  • Communicates with designated hospital staff caring for hospice patients in the inpatient setting regarding the Interdisciplinary Hospice Plan of Care and goals of the patient and family, hospice team and the attending physician.
  • Effectively documents all interactions with patients, families, and hospital staff in both the hospice and hospital EMRs.
  • Responsible for shared holiday and weekend coverage.

  • PREFERRED EDUCATION & EXPERIENCE

  • Valid Nursing License in New York State.
  • Valid driver's license in New York State.
  • Experience working in a fast-paced environment preferred
  • A minimum of two years' experience in home care or hospice preferred.
  • Experience and comfort in caring for terminally ill patients.
  • A basic understanding and commitment to the hospice philosophy.
  • WORK ENVIRONMENT / PHYSCIAL DEMANDS

  • Working conditions include working within assigned settings, including but not limited to : Hudson Valley Hospice offices, hospitals, patient / family homes, nursing homes, and assisted living facilities
  • Ability to carry up to twenty-five pounds of supplies and / or materials.
  • Routine duties involve potential for direct exposure to blood or body fluids.
  • Capability to work a flexible schedule to meet the needs of the patients including occasional weekends and holidays.
  • Ability to provide transportation for self and access public buildings to provide community education.
  • Ability to drive and work in winter weather including snow
  • Hours : 40hrs / week; (4) 10 hour shifts - Rotating Schedule with weekends and holidays included.

    Pay : $78,000-90,000 / year

    FLSA : Exempt
