### **Position Overview**
We are seeking an experienced **HIV Case Manager** to join our team in **Melbourne, Florida** and play a vital role in coordinating patient care, preventing care gaps, and fostering strong community relationships. Given our telemedicine-based approach, the ideal candidate will be **comfortable working with patients remotely** while also engaging in community outreach and in-person coordination when needed.
The ideal candidate will have **at least one year of experience in HIV case management in Florida** and a strong understanding of **the 340B Drug Pricing Program and grant writing.** Spanish-speaking applicants are highly encouraged to apply.
### **Key Responsibilities**
- Coordinate care with medical providers, social workers, and other healthcare professionals to ensure HIV-positive patients remain engaged in care.
- Work with **telemedicine platforms** to provide remote case management, ensuring continuity of care for patients in multiple states.
- Develop and implement strategies to prevent patients from being lost to follow-up and ensure their needs are being met.
- Assist patients in accessing and maintaining medical treatment, insurance coverage, and social support services.
- Work on **340B program initiatives** and collaborate with leadership on grant writing and funding opportunities to enhance patient resources.
- Engage with local organizations and community stakeholders to **identify new patients** and strengthen relationships within the community.
- Educate patients and community members on HIV prevention, treatment adherence, and available support services.
- Maintain accurate documentation, case notes, and patient records in compliance with federal and state regulations.
- Advocate for patients’ rights and ensure culturally competent care is provided to diverse populations.
### **Qualifications**
- **Minimum of one (1) year of experience in HIV case management in Florida.**
- Strong knowledge of **the 340B Drug Pricing Program** and experience with **grant writing.**
- Experience with **telemedicine-based case management** and comfort working with remote patients.
- Bachelor's degree in social work, public health, healthcare administration, or a related field (preferred).
- Experience working with underserved populations and a passion for community outreach.
- **Bilingual in Spanish is a plus.**
- Strong organizational, communication, and problem-solving skills.
- Ability to work collaboratively within a multidisciplinary team.
