What are the responsibilities and job description for the LEAD Case Manager position at Human Services Administration Organization?
Provides case management services to individuals involved in the Law Enforcement Assisted Diversion (LEAD) program. LEAD is a system-change initiative in which individuals can be diverted from arrest to long-term, hard-reduction-focused case management. The LEAD Case Manager, under the direction of the LEAD Supervisor, acts as a resource person, liaison, advocate, and coordinator for the individuals they will support in their assigned Cohort. Major responsibilities include assessing an individual’s needs, identifying client-driven goals, applying harm reduction principles in daily work, assisting/linking individuals to necessary supports, resources, treatment as requested by the individual, and collaboration with other members within the LEAD Cohort. Must develop a rapport with the individual serving as their advocate, supporting them on their goals and their chosen path. This employee will also perform the work in accordance with established regulations, policies, procedures, and supervisory review.
SKILLS AND KNOWLEDGE
QUALIFICATIONS
Bachelor’s degree and one-year direct service experience working directly with persons with behavioral health needs and/or persons involved in the criminal justice system. Or any equivalent combination of experience and training.
DUTIES AND RESPONSIBILITIES
- Conducts interviews utilizing the appropriate assessment practices and tools to obtain necessary information from the individual served to determine services and resources needed.
- Identify client-driven goals and implement actions to achieve goals
- Consistent utilization of harm reduction techniques to prevent overdose, transmission of infectious disease, improve overall health and minimize barriers to accessing physical, mental health and substance abuse treatment
- Assist the individual in locating and obtaining needed services.
- Implements and provides services based on individual need and client preference:
- Be the “go-to” resource for the individual served
- Assure there is cross system coordination of services and that services are being provided
- Be a persistent advocate for those served and give feedback on systemic problems
- Provide a consistent positive outlook and full inclusion in the community.
- Collaboration with law enforcement on diversionary options for individuals.
- Provide support and advocacy to individuals through court proceedings as needed.
- Maintains service records in identified systems and ensures all documentation and data-tracking is completed as required
- Manage a caseload and carry out work under the direction of the LEAD Supervisor and administrative personnel.
- Participation in ongoing meetings with system partners, including but not limited to the Operational Workgroup meeting.
- Collaboration with LEAD system partners, including law enforcement officers and department leadership, treatment providers, community resources, county and city government staff and LEAD Project Management staff.
- Monitoring of service delivery.
- Participate in ongoing training in the field.
- Perform other duties as assigned
- Collaborate, network and attend relevant events with community members in identified LEAD jurisdictions
- Meet with clients in various locations, including but not limited to, public spaces, client home, provider offices, jail, hospitals, shelters, etc.
- Demonstrate proactive and tenacious efforts to initiate and maintain communication with referrals and clients
SKILLS AND KNOWLEDGE
- Ability to work in a team-oriented atmosphere.
- Knowledge of Harm Reduction Strategies.
- Knowledge of addiction as a disease, available treatment options, including harm reduction treatment options.
- Knowledge of current social, economic, and health problems and available resources.
- Ability to work with people with a variety of physical, emotional, or mental disabilities.
- Ability to plan and organize records, reports, and maintain a caseload in an effective and timely manner.
- Ability to be a self-starter and creative in identifying solutions to problems; knowing when to inform supervisor when obstacles arise
- Ability to demonstrate cultural competence and work with people from a variety of diverse agencies, backgrounds, resources, and communities.
- Good communication skills.
- Familiarity with systems impacting the clients we serve, including but not limited to, behavioral health, criminal justice system, drug & alcohol and housing
- Strong time management skills and ability to pivot schedule, as needed
- Possess Acts 33/34 and FBI Clearances.
- Upon hire employee must obtain Institutional Clearances through the National Crimes Informational Center (NCIC) conducted by The Internal Affairs Dept of the Allegheny County Jail.
- Obtain yearly Institutional Clearances through the National Crimes Informational Center (NCIC) conducted by The Internal Affairs Dept of the Allegheny County Jail.
- Have a valid Pennsylvania motor vehicle license, access to a personal vehicle, and have maximum auto insurance on an employee driven vehicle.
- Ability to work independently.
