JOB TITLE: In-Home Case Manager - Psychotherapist

DEPARTMENT: Family Services

STATUS: Full-time, Non-Exempt

HOURS PER WEEK: 40

REPORTS TO: Program Manager

SALARY RANGE: $30 – 34 hourly

POSITION SUMMARY:

Provide home-based case management and psychotherapy services using evidence-based practices for families with children 0-5 years old. Services are designed to prevent child abuse and neglect and are provided in the client family home. Services will be provided at the agency and/or off-site when necessary.

DUTIES AND RESPONSIBILITIES:

  • Conduct home visits 2-4 times a month per family on caseload (12-16 cases), as identified on the service plan; develop a professional, trusting relationship with client families.
  • Provide home-based face-to-face case management, systems navigation/linkage services, general counseling, service planning, assessment, crisis intervention, and advocacy services with client family members on caseload; occasional on-site visits may be allowed.
  • Provide psychotherapy services, including groups, to families as needed.
  • Address client family mental health concerns, self-awareness, problem-solving, development of insight, unresolved trauma, parental awareness, and emotional health, etc.
  • Collaborate with the Department of Children and Family Services (DCFS) and other referral sources to coordinate services and service plan goals.
  • Review assessment and screening information with parents/caregivers, develop action plans, program goals, and review progress made on a quarterly basis or as required by program.
  • Assist client families in increasing and improving protective factors and reducing risks for or further child abuse and/or neglect.
  • Adhere to child abuse reporting mandates and legal and ethical standards.
  • Adhere to the DCFS Shared Core Practice Model and Strengthening Families Protective Factors Framework in the service delivery with client families.
  • Participate in trainings and certifications of evidence-based practice approaches.
  • Establish rapport, trust, and professional relationships with coworkers, supervisors, community, and stakeholders.
  • Participate in reflective supervision, clinical training, case conference, and consultation as required by the contract; and adhere to Board of Behavioral Sciences State regulations, if applicable.
  • Attend administrative meetings held by agency, program, community, or County, as required.
  • Attend peer review meetings and maintain quality assurance standards as per contract.
  • Establish and maintain a current and accurate record-keeping system. Ensure all family chart files are up to date and accurate.
  • Report to work at scheduled times, be open to flexible hours to meet the needs of the client families, and maintain good attendance.
  • Other duties as assigned.

KNOWLEDGE, SKILLS AND ABILITIES:

  • Knowledge of Strengthening Families Protective Factors and DCFS Core Practice Model highly desirable.
  • Knowledge of evidenced-based practices highly desirable.
  • A minimum of two years' experience working with underserved and unserved populations, such as the LGBTQ community, people of color, male survivors, and tribal populations is highly preferred.
  • Knowledge of community engagement, training, and core practice models aimed towards eliminating racial disproportionality, and disparity within communities is highly desirable
  • Knowledge of child development and child abuse risk factors such as domestic violence, substance abuse, mental health issues and/other factors that contribute to child abuse and neglect.
  • Knowledge of community resources available for families of young children.
  • Proven competency skills and the ability to exercise independent and good judgment.
  • Excellent oral and written communication and organizational skills.
  • Ability to follow policies, procedures, and legal and ethical standards; maintain confidentiality.
  • Ability to meet attendance standards and work hours, including evenings and weekend, as necessary.
  • Bilingual (English/Spanish) highly desirable.

EDUCATION AND EXPERIENCE:

  • Education: Master’s degree in Social Work, Marriage and Family Therapy, or a related field; registered with the Board of Behavioral Sciences is required.
  • Minimum of (2) years’ experience in social services or a family support program working with at-risk or high-risk families at risk of child abuse and neglect with children under the age of five years; minimum of (2) years' experience working with underserved and unserved populations, such as people of color, LGBTQ community, male survivors, and/or tribal populations.

LICENSING/CERTIFICATION REQUIREMENTS:

  • Driving is required on a daily basis. Must provide own vehicle, valid California driver’s license and proof of liability insurance (name must be on insurance card).
  • Negative tuberculosis screen (required).
  • Must pass criminal background check.
  • Certifications in evidence-based approaches for families with children age 0-5, highly desirable.
  • First Aid certification, Child CPR certification highly desirable.

  • The work location is in Bell Gardens, CA, with the majority of job duties performed in client families’ homes and off-site locations throughout the SPA 7 region. The Bell Gardens location has moderate noise levels and is a non-smoking environment.
  • Employee must climb up one flight of stairs and must occasionally lift and/or move up to 25 pounds.
