Human Services, Inc. was founded in 1972 as one of Chester County’s few community mental health clinics. During that time we are proud to have helped thousands of people treat and recover from various forms of anxiety, depression, bipolar disorder and schizophrenia, relationship problems, stress management and a host of other issues that plague many of us. We have 3 Outpatient Clinic locations in Chester County; one in Thorndale, Coatesville and Oxford.
The Blended Case Manager (BCM) position is full time, exempt position available in the Case Management Department located at Human Services Inc., 50 James Buchanan Drive, Thorndale, PA.
The Blended Case Manager is a closely supervised position that provides case management services to a maximum caseload of (30) individuals who meet the State criteria for Blended Case Management services. Services are provided in the office and on an community basis and are to be delivered utilizing Recovery concepts and follow all State, County and Agency regulations.
Summary of Job Duties:
- Identifying needed services across all life areas (housing, education, employment/vocational, income and benefits mental health treatment, drug and alcohol treatment, socialization/informal supports, medical, etc.)
- Linking identified services including housing, utility, vocational and mental health rehabilitation programs.
- Working collaboratively with internal Outpatient staff regarding treatment needs and updates for each participant.
- Responding to metal health crisis situations involving the participants on the BCM's case load as needed.
- Facilitating psychiatric hospitalizations including but not limited to meeting with the person who is experiencing the mental health emergency, if needed, contacting the person's insurance company to find an available bed and facilitate transportation for the person receiving services.
- Perform on-call responsibilities 3-4 times per month.
- Serves as an advocate for the person on their caseload when needed.
Employees are required to pass FBI/Child/Driver's license background checks. Employees must maintain a valid driver's license and current automobile insurance. Employee cannot be on the Federal ineligibility list.
Minimum Qualifications :
- Bachelor’s degree required in the human services field.
- Have a thorough knowledge of recovery and wellness philosophy.
- Knowledge of Chester County resources is preferred.
- Previous experience working with a forensic population preferred.
- Have a working knowledge of issues faced by those who experience serious mental illness and previous criminal histories (2 years direct service experience in the human services field is preferred).
- Must possess the ability to communicate effectively both orally and in writing.
- Must have strong organizational and time management skills.
- Establish positive working relationships with staff, community partners and the individuals you work with.
- Bilingual skills preferred
- Have the ability to recognize and be sensitive to cultural and ethnic differences.
- Must be able to safely perform essential job functions with or without reasonable accommodations.
