Status of Position: Full-time, Exempt available to qualified applicant in the Homeless and Support Services Department at Human Services, Inc. located in West Chester.
CYF Housing Case Manager is responsible for:
Completing the intake process for families being supported within the department. Review intake information, current housing issues, financial history, employment, education, psychiatric, substance abuse and medical histories, psychological history, and current psychological strengths and problem areas.
Develop and periodically review individual service plans for each client on the case load. Determine need for additional records and information and initiate processes to obtain such records and information.
Facilitate individual service plan that address client needs as determined by the service plan.
Function as an advocate of client to access appropriate services.
Provide initiative in networking and communicating with various service providers, cargeivers, landlords involved with the identified client.
Provide outreach services to client services to client and involved others that may include homes and community visits, and appropriate settings in which to meet with clients and involved others.
Document and Supervise Housing Initiative Services:
Provide and maintain appropriate documentation of all services rendered in the Housing Initiative Program.
Complete initial assessment within 30 days of intake and forward to CYF case worker. Complete monthly reported on all open cases and forward to appropriate County representative.
Review client status toward self-sufficiency/compliance with contract.
Budgeting and Payment Requests:
Collect household bills and income verification in order to complete initial budget.
When applicable, complete graduated rental payment worksheet.
Complete and process montthly payment requests by the 25th of each month for ongoing cases; as needed for all others. Other duties as assigned.
Minimum Qualifications :
- Bachelor's degree in human services or related course of study .
- Previous experience working with families in need.
- Previous experience with electronic medical records, excel and Microsoft
- Willingness to travel extensively throughout Chester County.
- Must possess the ability to communicate effectively both orally and in writing.
- Establish positive working relationships with staff, community partners and the individuals you work with.
- Relate positively with colleagues and representatives of other agencies.
- Have the ability to recognize and be sensitive to cultural and ethnic differences.
- Must be able to safely perform essential job functions with or without reasonable accommodations.
- Have an understanding of Recovery and Wellness philosophy
