Coaching for Success – Social Work Case Management
40 hours per week
Nonexempt position
Basic Functions: Participant Coach will work to develop a strong relationship with the participant, set short- and long-term goals, unify services and goals for less confusion, provide support, assist in removing barriers that hinder forward movement and maintain an up-to-date knowledge of all agency programs administered throughout assigned county. This individual must provide efficient and effective systems of accountability ensuring compliance in local, state, and federal regulations when delivering services to eligible participants. Must be knowledgeable of all community resources for information and referral.
Qualifications:
- Ability to effectively lead and connect with vulnerable populations.
- Experience in case work or family advocacy.
- Must have a valid Indiana driver’s license
- Must have a clean Driving Record
- Must pass Criminal History Background Check
- Must pass Pre-employment Drug Screen
Physical Demands: This position requires the ability to lift files, open filing cabinets, bend or stand, as necessary.
