Description
The Community Day Services Employee is responsible for implementing the Individual Service Plan Goals established for each client. The focus is to be on helping each client become and/or continue to be as independent as possible in their life activities. While other duties may be assigned, the primary responsibility of the Community Day staff is direct service to assigned clients.
ILLUSTRATIVE EXAMPLES OF POSITION DUTIES:
1. Provide each assigned client the training necessary to achieve his/her Individual Service Plan goals and program goals.
2. Conduct his/her assigned classes/training sessions as scheduled.
3. Maintain sensitivity to the service population’s cultural and socioeconomic characteristics.
4. Document all activities as directed Intuition, goals, or other forms.
5. Complete all needed paper work in timely fashion for services provided to clients.
6. Chart client attendance and job assignment as directed.
7 Chart each client's progress on goals each period.
8. Provide QIDP with information necessary to develop appropriate Individual Service Plans.
9. Complete Incident Reports of Accident, Illness or Adverse Incident as directed.
11. Prepare lesson plans and lead classes in accordance with the goals set forth in clients Individual Service Plans.
12. Complete other duties as are necessary or related to the successful completion of other stated position duties, or as mutually agreed.
This role is set for 40 hours a week, scheduled at 8:00AM to 4:00PM Monday through Friday

Requirements
1. High School diploma
2. Demonstrated ability to work effectively with people in a variety of professional relationships.
3. License to operate all agency vehicles
4. The ability to lift over 50 pounds.
