Position Overview: We are seeking an experienced Home Health Physical Therapist to join our dynamic team. The ideal candidate will be dedicated to improving the quality of life for our patients by providing expert physical therapy services tailored to individual needs. This role offers the opportunity to work autonomously while collaborating with interdisciplinary teams to ensure the highest standard of care.
Responsibilities:
- Conduct comprehensive assessments of patients' physical and functional abilities.
- Develop personalized treatment plans in accordance with physician recommendations and patients' goals.
- Provide skilled physical therapy interventions aimed at improving mobility, strength, balance, and overall function.
- Educate patients and their caregivers on therapeutic exercises, safety precautions, and injury prevention strategies.
- Monitor patients' progress and adjust treatment plans as necessary to optimize outcomes.
- Document all clinical findings, interventions, and patient responses accurately and in a timely manner.
- Collaborate effectively with physicians, nurses, and other healthcare professionals to ensure coordinated care and achieve desired outcomes.
- Maintain compliance with regulatory standards, including HIPAA and Medicare guidelines.
- Participate in ongoing professional development activities to enhance clinical skills and stay abreast of best practices in home health physical therapy.
Qualifications:
- Graduate of an accredited physical therapy program with a current license to practice in the state of Pennsylvania.
- Minimum of 3 years of clinical experience as a physical therapist, with a focus on home health care or skilled nursing facility preferred.
- Strong clinical reasoning and problem-solving skills, with the ability to adapt treatment plans based on individual patient needs.
- Strong clinical skills in assessment, treatment planning, and therapeutic interventions for a diverse patient population, including adults and older adults.
- Confidence is recognizing medical emergencies and responding appropriately.
- Excellent interpersonal and communication skills, with the ability to effectively collaborate with patients, families, and interdisciplinary team members.
- Demonstrated proficiency in conducting comprehensive physical therapy evaluations and developing evidence-based treatment plans.
- Monitor patients' progress, reassess goals, and modify treatment plans as necessary to ensure optimal outcomes and continuity of care.
- Supervise PTA
- Ability to work independently, manage time effectively, and prioritize tasks in a fast-paced environment.
- Proficiency in using electronic medical records (EMR) systems for documentation purposes. Kinnser Well sky or Homecare Homebase preferred.
- Experience with oasis preferred
- Reliable transportation and a valid driver's license required.
- BLS certification and other relevant certifications may be required.
- Recent TB screen or chest x-ray
