Responsibilities
- Conduct comprehensive assessments of patients' physical and functional abilities.
- Develop personalized treatment plans in accordance with physician recommendations and patients' goals.
- Provide skilled occupational therapy interventions aimed at improving mobility, strength, balance, and overall function.
- Educate patients and their caregivers on therapeutic exercises, safety precautions, and injury prevention strategies.
- Monitor patients' progress and adjust treatment plans as necessary to optimize outcomes.
- Document all clinical findings, interventions, and patient responses accurately and in a timely manner.
- Collaborate effectively with physicians, nurses, and other healthcare professionals to ensure coordinated care and achieve desired outcomes.
- Maintain compliance with regulatory standards, including HIPAA and Medicare guidelines.
- Participate in ongoing professional development activities to enhance clinical skills and stay abreast of best practices in home health occupational therapy.
- Graduate of an accredited occupational therapy program with a current license to practice in the state of Virginia.
- Minimum of 3 years of clinical experience as a occupational therapist, with a focus on home health care or skilled nursing facility preferred.
- Strong clinical reasoning and problem-solving skills, with the ability to adapt treatment plans based on individual patient needs.
- Strong clinical skills in assessment, treatment planning, and therapeutic interventions for a diverse patient population, including adults and older adults.
- Confidence is recognizing medical emergencies and responding appropriately.
- Excellent interpersonal and communication skills, with the ability to effectively collaborate with patients, families, and interdisciplinary team members.
- Demonstrated proficiency in conducting comprehensive occupation therapy evaluations and developing evidence-based treatment plans.
- Monitor patients' progress, reassess goals, and modify treatment plans as necessary to ensure optimal outcomes and continuity of care.
- Supervise COTAs
- Ability to work independently, manage time effectively, and prioritize tasks in a fast-paced environment.
- Proficiency in using electronic medical records (EMR) systems for documentation purposes. Kinnser Wellsky or Homecare Homebase preferred.
- Reliable transportation and a valid driver's license required.
- BLS certification and other relevant certifications may be required.
- Recent TB screen or chest x-ray
