We are seeking a highly motivated and compassionate Care Community Coordinator to join our team. The successful candidate will work closely with our clinical and non-clinical teams to ensure that our members receive the best possible care and support.
Job Description
- Coordinate with the healthcare team to develop and implement individualized care plans that meet the unique needs and goals of each member.
- Provide education and support to members and their families on various aspects of healthcare, including disease management, medication adherence, and healthy lifestyle habits.
- Collaborate with community organizations and providers to connect members with local resources and services that promote health and well-being.
- Conduct regular outreach and engagement activities to ensure that members are aware of available services and benefits.
