The LTSS Care Coordinator employs various strategies and techniques to address members' health issues. You will operate within established professional standards and procedures, making informed decisions about your approach to care.
Your responsibilities will include :
- Assessing care needs to ensure members receive services in the least restrictive settings for their optimal well-being.
- Identifying and resolving barriers to effective care.
- Planning and implementing interventions tailored to meet individual care needs.
- Coordinating services, monitoring, and evaluating case management plans against members' personal goals.
- Continuously monitoring patient care to ensure progress towards desired outcomes through assessments, data analysis, and active planning.
- Directing members and families to appropriate resources and facilitating interactions with providers and interdisciplinary teams.
- Conducting visits to Medicaid members in various settings, including homes and facilities, requiring 75-90% local travel.
