Location : Multiple FQHC Locations Available(Bronx, Downtown Brooklyn, Manhattan, Staten Island)
Schedule : Monday – Friday, 8 : 30 AM – 5 : 00 PM (40 hours per week)

HumanEdge is seeking experienced and compassionate Licensed Practical Nurses (LPNs) to join our team and support a Federally Qualified Health Center (FQHC) network in New York City. This is a floating LPN position , requiring flexibility to work across multiple locations as needed.
Our ideal candidate is a courteous, efficient, and patient-focused professional who can provide exceptional care to a special needs patient population and their caregivers in a dynamic outpatient setting. Key Responsibilities
- Coordinate and prepare daily assignments for nursing staff
- Assist in implementing systems for daily medical and nursing operations, in collaboration with the Director of Nursing
- Participate as a key member of the Patient-Centered Medical Home (PCMH) Care Team , attending daily huddles and practice team meetings
- Ensure compliance with all PCMH initiatives and standards
- Provide high-quality, patient-centered nursing care across multiple clinic locations
Active Licensed Practical Nurse (LPN) license in New York State
