In this role, you will work closely with a multidisciplinary team to deliver high-quality patient care, focusing on education, rehabilitation, and individualized treatment plans.
Key Responsibilities:
- Conduct comprehensive assessments within 8 hours of admission
- Develop individualized care plans in collaboration with patients and healthcare teams
- Deliver direct patient care, focusing on education and rehabilitation
- Ensure accurate and timely documentation of patient progress
- Support discharge planning from day one to promote continuity of care
- Communicate effectively with physicians, charge nurses, and interdisciplinary team members
- Maintain a safe and positive work environment
Requirements:
- Acute care or rehabilitation nursing experience preferred
