Responsibilities:

• Provide physical therapy services to patients in a Skilled Nursing Facility setting
• Develop and implement individualized treatment plans to meet the unique needs of each patient
• Conduct thorough evaluations and assessments to identify areas for improvement
• Collaborate with other healthcare professionals to ensure seamless care coordination
• Maintain accurate records and documentation of patient progress

Instructions:

The selected candidate will be expected to work 40 hours per week, 8 hours a day, with a start date as soon as possible and a duration of 13 weeks.
