Brooklyn Office
As an Intake RN - MLTC, you will assess potential new members for eligibility and explain the program to them and their families. You will play a crucial role in ensuring a seamless transition to managed long-term care.
Large MLTC located in Brooklyn, NY. Addtional offices located in the Bronx, Queens, and Manhattan. Conduct outreach activities within the community to assess the eligibility and safety of potential members.
Collaborate with the intake / Outreach team and clinical team to ensure appropriate services and provide new members.
Communicate effectively with potential members, families, referral sources and physicians. Current NYS Registered Nurse License
BSN required
One year of experience in Acute Care setting
Must have UAS-NY experience
Homecare and management experience preferred
Proficient with MS Office
Strong oral, written and interpersonal skills
Bilingual preferred but not a must MLTC or 1 year of Acute Care experience NYS State Nurse License INDWPIT #ZRWPIT $94,500.00 - $105,000.00

Salary : $94,500 - $105,000
