What are the responsibilities and job description for the Behavioral Health Case Coordinator position at HUMANIM INC?
Reporting to the Behavioral Health Program Manager, this role will be responsible for the full case management of the assigned caseload and coordination of appropriate services for clients. This position provides individualized teaching, training, and support needed to adults with severe and persistent mental illnesses to help them maintain their independence in the community. Case Coordinators will work with clients in their homes and throughout the community to provide life skills training and case management services. This is a unique opportunity to provide services in a program setting and in the community. The coordinator will assist with developing / facilitating groups / skill-building modules to support individuals recovery and with recreational outings to integrate individuals in their community. This role assumes responsibility for advocating for, coordinating the delivery of, and monitoring a wide variety of services and supports for eligible persons to enable people to lead meaningful and productive lives. Additionally, you will be working as part of an interdisciplinary team with opportunities for cross-training, development, and clinical supervision with the option of becoming a Certified Psychiatric Rehabilitation Practitioner (CPRP).
