Reporting to the Director Community Housing, the Residential Rehabilitation Program Manager oversees the coordination of care, rehabilitation planning, and proper documentation of all service delivery for everyone receiving Residential Rehabilitation Services. As part of a multidisciplinary team, the role will provide assessment, person-centered treatment planning other services driven by the client's needs. The Manager manages the intake process and delivery of services for the Residential Rehabilitation Programs. This role also assists in the implementation of an integrated service delivery system with other Humanim programs and various community organizations.
