What are the responsibilities and job description for the RN Case Manager/Delegating Nurse position at HUMANIM INC?
Responsible for working under our Director of Nursing to provide nursing services for the agencys programs that include assessing individuals health needs, developing care plans, visiting day programs, and supervising direct support professionals in implementing delegated nursing tasks and medication administration. In addition, acts as a liaison with physicians and other healthcare professionals, maintains appropriate nursing documentation including 45-day reviews, providing staff training, and participating as an integral member of each individuals support team. The RN Case Manager / Delegating Nurse is responsible for the delegation of and oversight of assigned nursing functions to agency staff that work in Humanims programs as well as the case management of individuals deemed necessary. This position is designated as an essential employee and provides essential care to ID / DD individuals in the community setting and is required to work during state and / or national emergency situations. 24 hours work week with potential remote work, supporting individuals within the Day ProgramEmergency / vacation coverage for residential delegating nurse as neededRotating phone triage
