Key Responsibilities:
Personal Care Assistance: Assist patients with daily activities such as bathing, dressing, grooming, and toileting, ensuring their dignity and comfort.
Mobility Assistance: Help patients with walking, transferring between bed and chair, and using mobility devices, ensuring safety at all times.
Meal Preparation: Prepare nutritious meals based on dietary requirements, assist with feeding if necessary, and ensure proper hydration throughout the day.
Medication Reminders: Provide reminders for prescribed medications and assist in setting up or organizing medications according to the care plan.
Light Housekeeping: Perform light housekeeping tasks, including tidying up the patient’s living area, laundry, and maintaining a safe, clean environment.
Companionship and Emotional Support: Provide companionship, engage in conversation, and offer emotional support to enhance the patient's mental and emotional well-being.
Monitoring Health Conditions: Observe and report changes in the patient’s physical or mental condition to the registered nurse or supervisor, including monitoring vital signs as instructed.
Documenting Care: Accurately record daily activities, patient progress, and any concerns or changes in condition, adhering to agency policies.
Assist with Exercise and Rehabilitation: Support patients in performing prescribed exercises or rehabilitation routines, as guided by healthcare professionals.
Family Communication: Maintain open and respectful communication with patients’ family members, providing updates on the patient's condition and care.
Follow Care Plan: Adhere to individualized care plans, ensuring patient-specific needs are met and providing high-quality care at all times.
