Please Read : We are urgently seeking Support Workers who can provide assistance and personal care to adults with disabilities. Join our team in providing quality care and assistance to individuals to be part of our wonderful community.

SUMMARY : The Support Worker provides monitoring, support, and encouragement to clients with disabilities living in their own homes or ready to transition to their own homes. Support Workers are required to provide transportation to client when need. They must be able to clear transportation requirements.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.

1. Implements objectives of the people we support with Individual Support Plans.

2. Provides emotional support, nutrition awareness, house finding, and money management training to people we support.

3. Utilizes community support services as appropriate.

4. Provides transportation to various appointments.

5. Maintains daily support notes.

6. Communicates with the support services program director, case managers, and other agencies as appropriate.

7. Attends meetings and training as required.

8. Lifts and transfers adults as required.

9. Assists people we support with self-help activities such as feeding, toileting, dressing, and personal hygiene.

10. Assists people we serve with special devices such as wheelchairs, walkers, prostheses, or braces.

11. Submits semi-annual progress reports on consumers.

12. Performs other job-related duties as required.

SUPERVISORY RESPONSIBILITIES :

Positions supervised : None

BENEFITS : Support Workers who work an average of 30 hours per week for six consecutive months during the six-month measurement period are eligible for health insurance. Support workers earned vacation which is pro-rated based on the number of hours worked.

At the time of hire, temporary on-call employees will receive 40 hours of sick leave that can be used after 90 days of employment. If the employee has used any sick leave hours during the year, the total will be brought back up to 40 hours at the beginning of January each year.

QUALIFICATIONS AND REQUIREMENTS : To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below represent the knowledge, skill, and / or ability required. Reasonable accommodations may be made to enable individuals with disabilities to perform essential functions.

EDUCATION and / or EXPERIENCE :

High school diploma or general education degree (GED) with at least six months of related experience with individuals with disabilities. College coursework in the field of social welfare, sociology, psychology, education, or social services is desirable. Also requires knowledge of community resources.

LANGUAGE SKILLS :

Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence.

Ability to speak effectively in one-on-one and small group situations with consumers, their significant others, HCAR staff, the regional center, other agencies, and community members.

MATHEMATICAL SKILLS :

Ability to add, subtract, multiply, and divide into all units of measure using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percent and to draw and interpret bar graphs.

REASONING ABILITY :

Ability to solve practical problems and deal with various concrete variables in situations with limited standardization. Ability to interpret various instructions furnished in written, oral, diagram, or schedule form.

CERTIFICATES, LICENSES, AND REGISTRATIONS :

First Aid and CPR certification, TB and fingerprint clearance, and physical exam. Transportation clearance.

OTHER SKILLS and ABILITIES :

Ability to prioritize job responsibilities and relate to individuals with disabilities. Ability to effectively support clients in finding suitable housing, relocating, budgeting, shopping, and managing households. Reliable transportation is also required.

While performing the duties of this job, the employee is frequently required to stand; walk; sit; stoop; kneel; use hands to finger, handle, or feel objects, tools, or controls; reach with hands and arms; and talk or hear. The employee is occasionally required to crouch or crawl and taste or smell.

The employee must frequently lift and transfer adults and lift / or move objects up to 50 pounds. Specific vision abilities required by this job include close vision, distance vision, peripheral vision, depth perception, and the ability to adjust focus.

While performing the duties of this job, the employee occasionally works in outside weather conditions.

The noise level in the work environment is usually moderate but can vary depending on consumer support needs and the location of the household.
