POSITION SUMMARY Directs, plans, organizes, and implements quality social services for hospital patients, nursing home residents, behavioral health patients, and families in accordance with Federal, State and local regulations and requirements. Works in collaboration with case manager/discharge planner, LCSWs, and the hospital staff to manage the patient needs for a safe discharge, transfer to another facility, or other needs.

JOB TITLE: Social Worker
Physical Demands:
- Frequent extremely heavy physical effort (lift, carry, push, pull 50 pounds or more)
