POSITION SUMMARY

Directs, plans, organizes, and implements quality social services for hospital patients, nursing home residents, behavioral health patients, and families in accordance with Federal, State and local regulations and requirements. Works in collaboration with case manager/discharge planner, LCSWs, and the hospital staff to manage the patient needs for a safe discharge, transfer to another facility, or other needs.

 

 

 

 

POSITION QUALIFICATIONS

Required Qualifications:

  • Licensed Master's degree-level clinician in Social Work, with 2 years of experience in behavioral health and/or Clinical Social Work Inter working towards completion of LCSW, licensure.
  • Licenses must be active and unrestricted.
  • Basic Microsoft Word skills

Preferred Qualifications:

  • 2 years of experience in a related mental health environment.
  • Case management experience.
  • Experience in managed care.

Primary Responsibilities for In-Patient ER/Hospital:

  • Assesses patient and family psychosocial needs and develops plan of care in concert with patient, physicians, nurses, and other members of the departmental team.
  • Links patients/family to appropriate community resources including but not limited to, information/referral to sources of financial assistance, transportation, support groups, and other community services.
  • Documents patient/family status, diagnosis, treatment plan, goals, interventions, evaluation results, observations, and progress in medical record.
  • Serves as patient advocate and liaison with physicians, families, insurance company, community agencies and others as needed to ensure continuity of care.
  • Provides discharge planning based on meeting identified goals of the treatment plan anywhere.

  • Links patients/family to appropriate community resources including but not limited to, information/referral to sources of financial assistance, transportation, support groups, and other community services.
  • Documents patient/family status, diagnosis, treatment plan, goals, and interventions, evaluation results, observations and progress in medical record.
  • Serves as patient advocate and liaison with physicians, families, insurance company, community agencies and others as needed to ensure continuity of care.
  • Provides discharge planning based on meeting identified goals of the treatment plan anywhere.
  • Complete all notes in the prescribed timeline.
  • Comply with all regulations.
  • Develop and coordinate therapeutic program services on the behavioral health program unit.
  • Educate patients and families on signs and symptoms of illness to prevent re-hospitalization and improve disease management skills.
  • Ensure the clinic operates within all regulations.
  • Provide social service functions in the behavioral healthcare setting, including assessments, treatment planning, discharge planning, and case management to patients and families.
  • Provide the best possible patient care.
  • Serve as a resource person to healthcare professionals concerning psycho-social problems and community resources.
  • Work with ethics, kindness, and accountability to enhance the quality of life for our patients.

The CSW-I applicant must be able to demonstrate:

 

The ability to diagnose, assess, and treat mental and emotional conditions

Comprehensive psychosocial assessment including knowledge and utilization of Mental Status Exams (MSE) Determination of diagnosis, i.e. use of the DSM

Development of treatment plans with specific goals

Various clinical intervention approaches

Document and review of treatment outcomes

Knowledge of psychopharmacology

Knowledge of addictions and the related clinical interventions

Suicidal/homicidal evaluations and interventions

Abuse/neglect evaluations and interventions

Experience with a range of clientele

DESCRIPTION OF ESSENTIAL PHYSICAL DEMANDS AND WORKING CONDITIONS

JOB TITLE: Social Worker

Physical Demands:

Working Conditions:

  • Contact with patients under wide variety of circumstances
  • Exposed to unpleasant elements (accidents, injuries, and illness)

  • Requires judgment/action which could have a significant impact on a patients life
  • May be exposed to all types of weather and temperatures

Required Protective Equipment:

  • Gown, medical gloves, face shield as necessary and other equipment as required by OSHA regulations and hospital policy.
