What are the responsibilities and job description for the Master of Social Work (MSW) Long Term Care position at Humboldt House Rehabilitation and Nursing Center?
Our Facility has an opportunity for a Social Worker who embodies these core values.
Under the direction of the Director of Social Services and / or Social Work Consultant, assists with determining the long range and short range goals for residents of facility as related to their social, emotional and financial needs.
Administrative / Resident Care Responsibilities :
Provides orientation to long term care facility and services for residents and family members.
Promptly follows-up on admission inquiries, evaluates and assesses the needs of prospective residents to determine appropriateness of facility placement.
Schedules appointments with resident, responsible party, guardian, next of kin or sponsoring agency to :
Review and interpret Bill of Rights, including facility Policy and Procedure implementation. Obtain signatures, sign as a witness to signatures, and provide resident or above-mentioned representative with copies.
Obtain signature from resident authorizing release of medical records from referring agency or responsible party.
Obtain signature and sign as witness as per form of facility “Authorizing the Release of Hospital and Medical Records” when indicated.
Attend and lead Patient Care Conference Meetings within 21 days of resident admission.
Demonstrates responsibility for resident transfers to the following :
Hospitals :
Notifies all disciplines if resident is recipient of Medicaid funding if resident’s condition does not warrant a return within 15-20 day period.
Community :
Obtains current release of information.
Conducts a discharge planning conference at the discretion of the planner.
Issues pending discharge bulletin for resident.
Formulates post-discharge summary.
Intra-Facility Discharge :
Completes a discharge summary.
6. Attends Patient Care Conference meeting.
7. Performs various Quality Assurance audits.
8. Monitors departmental budget and reports expenses per facility policy to the Administrator.
9. Works with the Administrator in maintaining established activity standards.
