Especially Looking for Candidates with Skills, Knowledge, and Experience in:

- Have a background in human services, psychology, or social work
- 6 months or more of supervisory experience
Minimum Qualifications:
- Must be eligible to work in the United States
- Must speak fluent English
- Must be 21 years or older
- At least one year of experience working directly with persons with intellectual disabilities or other developmental disabilities.
- Is either a physician and surgeon, or an osteopathy physician and surgeon, a registered nurse, or a human services professional possessing at least a bachelor’s degree in a human services field, including but not limited to sociology, special education, rehabilitation counseling or psychology, nursing, social work, human development, health science, public health, or other behavioral or social science.
- Must be able to pass state and federal background checks
- Can Demonstrate Basic Computer Skills
As a Case Manager, your primary responsibility will be to ensure the individuals' health, wellness, and safety needs are being met.
Other Responsibilities Include:
- Support and guide our individuals by identifying resources and needs, then developing tailored plans to increase their self-sufficiency.
- Adhere to all required company, state and federal policies, procedures, and guidelines.
- Accurately document case notes for all allowable communication/interactions, observations, and activities related to the individuals receiving services from HHDS.
- Face to face and phone contact with the individual and his/her direct support staff to ensure the individual’s health, wellness, and safety needs are being met.
- Ensure all individuals receiving services from HHDS experience the highest quality of life available through supports/services.
- Ensure all necessary and/or required paperwork for individuals on their caseload is completed e.g., Medicaid, SSI/SSA, SNAP, applications, redeterminations, etc.
- Interview, hire, supervise, train, discipline (when necessary) and provide written evaluations on the direct support staff under your supervision.
- Review monthly documentation.
- Review and approve direct support staff schedules.
- Act as an individual’s “Representative Payee” with Social Security when required.
- Actively participate in annual PCP (Person Centered Plan), quarterly and other meetings (as needed) for individuals on their caseload.
- Adhere to and comply with all HHDS and RRC policies, procedures, and trainings.
- Other duties as assigned.
