What are the responsibilities and job description for the Acute Care Physical Therapist - Registry/Per Diem position at Humboldt Park Health?
Position Highlights:
Performs Physical Therapy (PT) evaluation, reassessment and consultation; provides physical therapy plan of care, goals, discharge planning, patient and significant other (SO) education; renders physical therapy services, interventions, use of therapeutic modalities within the practice guidelines of the American Physical Therapy Association (APTA) and other regulatory agencies; and participates in collaborative patient care services.
Responsibilities:
- Provides patient care on age-specific populations as demonstrated by competency
- Performs PT evaluation, reassessment, and consultation upon referral of a physician, dentist, or podiatrist
- Provides plan of care, problem list, treatment goals, patient and family education and discharge planning
- Provides physical therapy intervention, procedures, services within the practice guidelines of the APTA and other medical regulatory agencies
- Participates in patient care plans and interdisciplinary discharge planning/continuum of care.
- Ensures patient safety, patient rights, confidentiality, and compliance to the standards of patient care, JCAHO guidelines, and other regulatory agencies
- Identifies areas and opportunities for performance improvement (PI) and demonstrates knowledge of the PI functions
- Functions as an advocate for the patient and demonstrates excellent customer relations.
- Ensures the provision of efficient effective and quality patient care services
- Attends mandated meetings and in-services
- Pursues continuing education and advancement in clinical knowledge and skills.
- Communicates with the physician regarding patient evaluation results, proposed treatment plans, and goals, discharge plans, progress updates, or changes in the patient’s condition
- Follows the hospital’s mission of “Patient First” and “Best Patient Experience”
- Follows the department-specific policies and procedures, maintains an acceptable attendance record
Required Qualification:
- Current Illinois Physical Therapy license
- Graduate of an Accredited Physical Therapy program
- Demonstrated competencies and clinical skills
- Mature, resourceful; commitment to “Patient First” and safe, quality patient care; team player, personable and dynamic with the ability to work in a diverse cultural setting
- CPR Certified
Physical Demands:
- Ability to perform physical requirement of lifting, bending, patient transfers and all physical therapy related work maneuvers.
- Able to handle the challenges of customer relations
